If you would like assistance verifying benefits, you may contact our office or email a copy of the front and back of your insurance card along with your date of birth, and we can help confirm participation and basic coverage information.
In-Network Insurance:

- Blue Cross Blue Shield
- Aetna
- Cigna

If your insurance plan is out of network, you may choose to receive care on a self-pay basis. Upon request, we can provide a detailed receipt (superbill) that you may submit to your insurance company for potential reimbursement, depending on your plan’s out-of-network benefits.

After insurance processing the claim (if applicable), you will have any overpayment refunded iand any underpayment will be charged to the card on file. We will make every effort to inform you of any large estimated appointment fees (e.g., over $100); however, it is ultimately the patient’s responsibility to understand terms of their insurance coverage. We accept the following forms of payment: debit, credit, FSA/HSA, and ACH.
No Show Policy:
In order to make care readily available for those in need, we ask that you provide 24 hours notice prior to your appointment for any reschedule or cancellation. As such, we charge a $75 cancellation fee for cancellations or reschedules within 24 hours of scheduled appointment and for any no-shows.
